
Aspire ADHD is committed to providing clear information about fees, insurance billing, and patient financial responsibilities. Please review the following financial policies before scheduling or receiving services.
Aspire ADHD participates with many major insurance plans. Participation with an insurance plan does not guarantee payment or coverage for any service.
As a courtesy, we will submit claims to participating insurance plans for eligible covered services.
Patients are responsible for understanding their individual insurance benefits, including:
Any amount determined by your insurance company to be the patient's responsibility is due according to the terms of this financial policy.
IVA-2 computerized testing is a separate, non-covered service and is not billed to insurance.
Payment is required to reserve your testing session.

Copayments, deductibles, coinsurance, self-pay fees, non-covered services, and other known patient balances are due at the time of service unless a written payment arrangement has been approved in advance.
Aspire ADHD accepts approved forms of electronic payment.
Aspire ADHD require patients to maintain a valid credit or debit card on file.
Your card may be charged for patient-responsibility balances after insurance processing, copayments, deductibles, coinsurance, non-covered services, cancellation or no-show fees, and other charges authorized under our financial policies and patient agreements.
Patients are responsible for maintaining current payment information.
Insurance coverage is an agreement between the patient and the insurance company.
Verification of benefits or eligibility is not a guarantee of payment.
Patients are responsible for providing accurate and current insurance information before receiving services.
If an insurance claim is denied, rejected, or not paid because of inaccurate information, inactive coverage, failure to obtain a required referral or authorization, or another patient-related issue, the patient may be responsible for the balance.
Patients are responsible for paying balances according to statements and notices provided by Aspire ADHD.
Patients with outstanding balances may be required to resolve the balance or establish an approved payment arrangement before scheduling additional non-urgent services.
Unpaid balances may be referred to a collection agency or other lawful collection process after appropriate notice.
Patients who are not using insurance are responsible for payment of all services according to Aspire ADHD's current self-pay fee schedule.
Payment is due at the time of service unless other arrangements have been approved in writing.
Self-pay patients may request a Good Faith Estimate of expected charges in accordance with applicable federal law.
Aspire ADHD accepts eligible Health Savings Account (HSA) and Flexible Spending Account (FSA) payment methods for qualified healthcare expenses.
Patients are responsible for determining whether a service qualifies for reimbursement under their individual HSA or FSA plan.
Financing through CareCredit may be available for eligible out-of-pocket healthcare expenses.
Approval, financing terms, promotional offers, and repayment requirements are determined by CareCredit.

Overpayments and account credits will be reviewed after insurance claims have been fully processed and all outstanding balances have been resolved.
Eligible refunds will be issued to the original payment method whenever possible.
Refund processing times may vary.
Patients are responsible for any fees or costs associated with returned payments, rejected transactions, or improperly disputed charges when permitted by law.
Initiating a payment dispute does not eliminate responsibility for valid charges owed under Aspire ADHD's financial policies and patient agreements.
Cancellation, late cancellation, and no-show fees are described in our Office Policies.
These fees are generally not covered by insurance and are the patient's responsibility.

By scheduling and receiving services from Aspire ADHD, patients acknowledge responsibility for charges associated with services received in accordance with Aspire ADHD's financial policies and patient agreements.
